Goodwill Industries of the Heartland offers Case Management Services designed to help meet people’s non-work
needs and remove barriers so that they may live and integrate
fully into their communities.
Case Management
Goodwill’s Case Management services are designed to assist
individuals with brain injury with a successful re-entry into the
community after injury. This service is offered for residents of
Benton, Cedar, Linn, Iowa, Jones or Washington Counties.
The purpose of Case Management is to promote the greatest level
of independence for individuals with brain injury and to empower
individuals and their families to plan for future needs.
Case Management provides assessment and referral services for
individuals and their families. Current needs, as well as personal
interests and goals are identified. An individual’s needs
are assessed in a variety of life domains, including financial,
medical, legal, residential, vocational/academic and social/personal.
Once needs are identified and prioritized, individuals are linked
with the appropriate services. Services provided by multiple agencies
are monitored and coordinated. Each case is monitored to assess
satisfaction and progress toward goals and to address additional
needs or goals as needed.
Case Management also provides advocacy and community education
for individuals with brain injuries and their families.
